ICD/SNONED codes added by a physician to a medical record have provenance, clinical governance and are part of the legal recorded clinical care
Tbe digital codes generated in this way are a source of trusted information- for the qbocexreasobs. It would be easy to link the clinically generated codes automatically to generic explanatory information that has the same code attached to it - without the need for a clinician.
