Dear Elon Danziger,

Thank you for your message about clinical guidelines, noting 'how often the guidelines themselves have gaps, ambiguities and even significant errors that are only revealed when you try to apply them at the bedside'.

You mentioned that you are using 'hazard analysis' to address these issues.

Please can you say a bit more about this, perhaps with a link to a description of how this approach is used?
