Neil, you raise an important question that (a) identifies the “anomalies” in the traditional “individual disease based “paradigm (Safe Care for NCDs) of our Spotlight Discussion and (b) may be more effectively addressed within the transformative context of a shifting paradigm of a whole of community and society approach (Safe Care for life).

Despite our references to “patient” and “safety” as seemingly generic concepts, patients globally are neither homogeneous or monolithic.

The varied “life" factors of individuals – as recorded in the SDGs - intersect in every individual's “wellbeing” and impact the contexts of our personal and collective health/care safety as well as harm. The paradox of Unsafe yet appropriate care as well as unpreventable harm emerges from systemic disregard of values of pluralism (the notion of appropriate being considered exclusively) included in the spirit of the Ubuntu philosophy.

The typical LMIC constraints or HIC advantages – for preventable harm and safe care are often mirrored in pockets of certain communities in both settings, the only difference being in the nature of their intra or inter- sectional life experience of benefit or handicap.

Listening to a TV interview of Dario Amodei, CEO of Anthropic, I hear three important words - Guard rails, accountability and external evaluation - addressing possible solutions for unpreventable and appropriate harms of AI security. Perhaps, they may also offer some timely insights to resolving the Patient Safety paradox?
