Thank you for the feedback.

I agree with you that Medical error is also a health system issue. Nevertheless, individual failure should be emphasized; else professionals will shift the responsibilities (also shared between the parties) and thereby miss the point. Without a formal scientific rigorous study, I will state that medical error is much more an individual failure that system issue.

An author even recommend that health professional should often discuss their errors among peers in as much as possible. I agree with the authors of the report "To Err is Human"; we should learn from our mistakes.
