Have you taken a look at the Rehabilitation section of the WHO website? One of the first items that drew my attention was:

The WHO Rehabilitation in health systems: guide for action is a set of resources that support countries to strengthen the provision of quality rehabilitation at all levels of the health system. The guide is being rolled out in several countries.
